Evaluate need on ADL training, proper use of Durable Medical Equipment, establish Home Exercise Program and Functional Mobility.

Speech Language Pathology

Assessment/evaluation on Cognitive, Speech, Swallowing, and recommended interventions.

MSW

To evaluate and provide necessary community resources according to client needs.

 

CHHA

To assist with ADLs and some iADLs

 

Skilled Nursing Services

SN/RN for case management of home health services. Demonstrate proper ways to control infections and basic home safety.
